The Work Activity Report Form SSA 821 is used to report your work activity if you are receiving disability benefits. This form is required to be completed if you have worked or are working while receiving disability benefits. The information collected on this form will be used to determine if your disability benefits should continue or if your benefits should be adjusted.

The completed form should be submitted to the Social Security Administration.
