Form SSA-671 - Sep 2015

The SSA 671 form is used to ensure proper social security benefits are given for railroad industry employment and to facilitate any required coordination with the Railroad Retirement Board.

Form SSA-671 should be filled out by claimant or claimant's spouse who have worked in the railroad industry and are applying for Social Security benefits.

Completed form should be submitted to the Social Security Administration office.
