Form 9465 - Feb 2017

Form 9465 is used to request an installment agreement if you cannot pay the full amount of taxes you owe. You must include a proposed monthly payment amount and the date you wish to make your payment each month. There is a fee to apply for an installment agreement, and interest and penalties will continue to accrue on the unpaid balance.

Individuals who owe taxes and cannot pay the full amount at once need this form.

The completed Form 9465 should be filed with the Internal Revenue Service Center where the corporation or trust files its income tax return.
