Form 9465 - Dec 2009

Form 9465 is used to request a monthly installment plan if you cannot pay the full amount of tax you owe within 120 days. The form requires information such as the amount you owe, the amount you can pay each month, and your contact information. The form must be submitted along with your tax return or at least 10 days after receiving a notice from the IRS.

Taxpayers who are unable to pay their balance due in full within 120 days.

The completed Form 9465 should be submitted to the Internal Revenue Service.
