Drug Test Consent Form

A consent to drug testing form​ is a document that allows an employer, healthcare provider or organization to perform a drug screening on an individual. It confirms that the individual agrees to the testing, understands the procedures and permits the release of results to the requesting party. These forms are often used during hiring, after incidents or as part of regular workplace policy.

This form is completed by employees, job applicants or individuals required to undergo a drug test. It is requested by employers, government agencies or healthcare providers.

Submit the signed form to the requesting employer, HR department or medical testing provider. A copy may also be kept in the individual's personnel file for documentation.
