Performance Counseling Form Template

The Performance Counseling Form documents formal sessions between a supervisor and employee to address performance, attendance, conduct or policy concerns. It includes sections for issue description, prior warnings, discussion, an agreed action plan, follow-up dates and acknowledgement.

The Performance Counseling Form is completed by the supervisor or manager leading the counseling session, with contributions from the employee and optional HR oversight.

Once signed, the Performance Counseling Form is generally provided to the employee, stored in HR records and retained by the supervisor for follow‑up.
