The filer enters name and Social Security number, household/tax family size, modified AGI, household income, and federal poverty line details. Using figures from Form 1095-A, the form computes annual or monthly premiums, the applicable SLCSP premium, contribution amounts, PTC allowed, advance payments, and any net credit or repayment limitation. It also includes sections to allocate policy amounts with another taxpayer and an alternative calculation for year of marriage. The completed form is attached to Form 1040/1040-SR/1040-NR for filing.
