Form 945-X - Feb 2014
Form 945-X is used by businesses to correct errors on Form 945 that was previously filed. The form must be filed separately for each tax period that needs to be corrected. The form requires information such as the name and address of the business, the tax period being corrected, the corrected amounts, and an explanation of the errors. Once completed, the form must be mailed to the Internal Revenue Service.
