Consent for Release of Information - Feb 2023

Form SSA-3288 is used to obtain a beneficiary's consent to release information to a third party. The form requires information about the beneficiary, the third party, and the information to be released. The beneficiary must sign and date the form before submitting it to the local Social Security Administration office.
