W-9 Questionnaire - Mar 2014

The W-9 Questionnaire is used to collect information from contractors who provide services to the New Jersey Department of Military and Veterans Affairs for tax reporting purposes. It contains basic information about the contractor, such as the name, address, and taxpayer identification number. This form must be submitted to the New Jersey Department of Military and Veterans Affairs.

Vendors/payees who intend to do business with the State of New Jersey or New Jersey State employees who are seeking reimbursement for travel or training expenses.

The completed W-9 Questionnaire form should be submitted to the Office of Management and Budget (OMB).
