State Tax Form 128 is used to request a reduction in the amount of tax assessed due to an overvaluation or misclassification of the property. The form must be filed with the local Board of Assessors within three years of the date the tax was assessed. To request an abatement, fill out the form and submit it to the local Board of Assessors.

Individuals or businesses who believe that their real property tax or personal property tax has been overvalued or misclassified and want to request an abatement.

The completed State Tax Form 128 should be submitted to the local Board of Assessors.
