W-9 Form - Sep 2015

The W-9 Form is used to request the taxpayer identification number (TIN) of a U.S. person (including a resident alien) and to request certain certifications and claims for exemption. Any U.S. person or resident alien who is requested to provide their TIN to another person or entity for tax purposes should fill out the form and provide it to the requesting party.

The completed W-9 Form should be provided to the person or entity requesting the TIN.
