It requests the organization’s name, address, EIN, contact details, tax-exempt status, legal form, year formed, principal officer, and gross receipts. It then requires reporting of mission and program accomplishments, governance and policy disclosures, required schedule checklist responses, compliance questions, compensation of officers/key employees/contractors, detailed revenue and functional expenses, balance sheet amounts, and reconciliation of net assets. The return is open to public inspection and warns not to include Social Security numbers. An officer signature and date are required, and a paid preparer section may also require signature.
