Dog Permit Form - Sep 2020

The form is used by residents of Albany to apply for a dog permit to keep more than two (2) dogs, but at most four (4) dogs, at one address. This fillable form allows applicants to enter their personal information, dog details, and payment information. Once completed, it can be submitted to the City of Albany Police Department. The department will review the application and issue the dog permit if all requirements are met.

Any person, family, or business wishing to keep more than two (2) dogs, but not more than four (4) dogs, at one address.
