Form 4506-C is the IVES request for transcript of tax return used to authorize an authorized Income Verification Express Service participant such as a lender or other third party to request tax return transcript information directly from the Internal Revenue Service. The form includes the taxpayer’s identifying information the participant’s details the types of transcripts requested and the tax years or periods for which the transcripts are being requested.
