A Vendor Performance Review is a structured evaluation tool used to assess and score vendor performance across key business criteria during a defined review period. The form captures vendor information, applies a 1–5 rating scale across categories including quality, timeliness, cost management, compliance and communication, and produces a total score that can be benchmarked against other suppliers. Completed evaluations support contract decisions such as renewals, probation, scope expansion or vendor replacement. The form includes space for a written assessment, corrective action plan and sign-off by the relevant procurement, operations and management staff.
