State of Michigan Voter Registration Application

The State of Michigan Voter Registration Application form is used by residents of Michigan to register to vote or change their address. To register, you must be a resident of Michigan and the city or township where you are applying, a citizen of the United States and at least 18 years old by election day. The form requires you to provide your residential address and you can also provide a mailing address if you prefer to receive mail related to your voter registration or driver license at a different address. Once completed, the form should be submitted to your city or township clerk.
