Form W-4 is an employee's withholding certificate used by employers to determine the amount of tax withholding to deduct from employees' wages. Employees complete this form to inform their employer of their tax situation and to ensure the correct amount of tax is withheld. The form requires information such as the employee's name, address, filing status, allowances, and additional income.

All employees who receive wages subject to tax withholding.

The completed Form W-4 should be submitted to employer.
