City of Moreno Valley Photo/Video Release Form - May 2015

This form is used to obtain permission from individuals to use their image or likeness in photos, videos, or other media. This form should be filled out by the individual being photographed or recorded, or their legal guardian if they are minors. Once completed, the form should be submitted to the organization or individual requesting the use of the image or likeness.
